A referral directs the need to an appropriate service and may still require an interview, assessment, waiting or separate form. IFSCs work by residential locality; hospital medical social workers handle relevant medical needs. Being referred to a unit does not establish eligibility for every assistance programme it deals with.
From enquiry to follow-up
- Prepare three points: what happened, who is affected now and what help is sought. Add residence and callback availability, distinguishing immediate food or care gaps from a general eligibility question. This gives the receiving service something specific to assess rather than a broad request for all available benefits.
- Use the official hotline or service contact and explain language or access needs. If a worker already follows the case, provide the reference and new circumstances. Avoid placing complete identity and bank details in a public post while waiting for an unknown intermediary to offer help.
- Establish the receiving unit, whether it will contact you, the follow-up date and any consent needed. Record any appointment you must make yourself. Prepare evidence against that unit's requirements and do not treat the referral call as completion of every application or assessment stage.
- Follow up the same case at the agreed point with the unresolved need and new evidence. If the situation deteriorates while waiting, make contact again promptly. Immediate danger uses emergency services separately rather than waiting for a benefit assessment or a routine callback that was arranged earlier.
Limits to understand before applying
A hotline offering information does not mean identical immediate social-work service at every hour. Follow the stated duty and transfer arrangements. Enquiries, complaints, referrals and formal benefit applications can use different channels, so clarify which stage applies to the request.
Ask for help understanding a form, while keeping confirmation with the appropriate applicant. Report income, assets, household and address changes as required. A worker having introduced the service does not create automatic renewal or remove the need for later supporting evidence.
A practical support example
A call could say: 'I live in Kwai Chung; the caregiver entered hospital last week and an older relative lacks care this week. An existing centre follows the case. Please advise temporary support or referral; I can take calls this afternoon.' Record the receiving unit and follow-up date afterwards.
Explain the need and urgency, then record who handles each next step after referral.
